MDCAT Syllabus 2026: Full Breakdown and Marks Distribution

The syllabus is the one document that every MDCAT candidate should read before they buy a book, join an academy or write a study plan. It is the definitive statement of what can be asked, and almost every wasted week in a preparation year traces back to somebody not having read it. This guide explains what the 2026 syllabus contains, how the marks are distributed, and how to convert it into a study schedule.

One point before we start. The syllabus is issued by the Pakistan Medical and Dental Council and administered provincially. It has been revised more than once in recent years, and revisions are not always widely publicised. Always download the current version from the official source and treat any summary, including this one, as secondary.

Marks distribution for 2026

The paper consists of 180 multiple-choice questions to be attempted in 180 minutes, giving exactly one minute per question. The structure remains biology-heavy.

Read that table strategically rather than descriptively. Biology alone is worth more than chemistry and physics combined, which means the marginal hour spent on biology is usually worth more than the marginal hour spent anywhere else. English and logical reasoning are only eighteen questions between them, but they are also the most finite and the quickest to improve, which makes them unusually efficient targets late in preparation.

What each section covers

Biology

The biology component spans biological molecules and enzymes, cell structure and function, bioenergetics including photosynthesis and respiration, the diversity and classification of organisms, plant and animal physiology across the digestive, circulatory, respiratory, excretory and immune systems, coordination and control including nervous and hormonal mechanisms, reproduction and development, inheritance and molecular genetics, evolution, ecology, and biotechnology.

Questions favour precise recall and the identification of a process from a distinguishing feature. Vocabulary discipline matters more here than in any other section.

Chemistry

Chemistry divides into physical, inorganic and organic. Physical chemistry covers atomic structure, chemical bonding, states of matter, thermochemistry, equilibrium, reaction kinetics, solutions and electrochemistry. Inorganic chemistry centres on periodic classification and the properties of the main groups and transition elements. Organic chemistry covers the fundamental concepts, hydrocarbons, the major functional groups and their reactions, and macromolecules including the biologically relevant ones.

Physics

Physics covers force and motion, work energy and power, rotational and circular motion, waves and oscillations, thermodynamics, electrostatics, current electricity, electromagnetism and electromagnetic induction, optics, and modern and nuclear physics. Numerical work is generally lighter than at board level but the time pressure is much greater.

English

The English section tests usage rather than literature: grammar and sentence structure, tense and agreement, prepositions and articles, vocabulary in context, and short comprehension. It is entirely learnable and it is regularly neglected.

Logical reasoning

This section covers critical thinking, logical deduction, cause and effect relationships, analogies and sequence questions. Most pre-medical students have never studied this formally, which is precisely why a small amount of targeted practice produces disproportionate improvement.

How to download the official syllabus

1. Go to the official website of the Pakistan Medical and Dental Council, or of the provincial admitting body conducting your test.
2. Open the admissions or MDCAT section and look for the syllabus notification for the current year.
3. Download the PDF and check the issue date on the document itself. A file circulating in a study group with the date removed is not a reliable source.
4. Print the contents list. You will refer to it constantly, and a printed copy you can annotate is more useful than a file you have to open.

Turning the syllabus into a study plan

1. List every topic on a single sheet and mark each one as not started, covered, or tested. Three states only. Complexity here defeats the purpose.
2. Allocate time in proportion to weighting, which in practice means biology runs continuously throughout your preparation rather than in a block.
3. Move a topic from covered to tested only when you have attempted questions on it under timed conditions and reviewed your errors. Reading a chapter is not coverage.
4. Reserve the final six weeks for mixed full-length practice rather than topic work.
5. Re-check your topic list against the official document once more before the final month, in case a revision has been issued.

Checking your books against the syllabus

Before relying on any commercial book, open its contents page beside the official syllabus and mark three things: chapters in the book that are no longer examinable, syllabus topics the book does not cover, and any change in section weighting since the book was printed. This five-minute exercise is the single highest-value thing you can do at the start of preparation, and it is the main practical reason to avoid scanned copies of unknown vintage.
